Years 1 to 6
For student enrolment at Marian State School, Parents/Guardians - please fill out the enrolment forms held at the school office. PDF copies can be downloaded from below, filled out at home and then brought to the office. If you cannot download the document/s linked on this page, please contact us and we will provide a copy via the school office.
Please complete and return all documents along with:
1. Original Birth Certificate
2. Proof of Residency
( Two documents must be supplied - a Primary source and a Secondary source)
i. Primary must be one of the following:
Rates Notice or Lease Agreement or Unconditional Contract of Sale
ii. Secondary must be one of the following:
Ergon Electricity Account or Gas Account
Once we have received all documents, the school office can book an enrolment interview time for you and your child. An interview is required with the Principal or Deputy before the student starts.
